2023 Player Registration Form

You are registering your child for Ignite FC (boys) or Rebel Elite (girls) for participation for the 2023-2024 seasonal year.  Rebel Elite is a dba of Ignite FC.  As used herein, the term “Ignite FC” shall mean and refer to both Ignite FC and Ignite FC doing business as Rebel Elite.

I hereby voluntarily permit my child to participate in Ignite FC soccer activities, including participating in training, matches, scrimmages, promotional events and other similar pursuits (collectively the “Activities”).

I UNDERSTAND AND FULLY ACCEPT THAT THERE ARE RISKS INVOLVED IN SPORTS, AND THAT ACCIDENTS AND INJURIES ARE POSSIBLE AND CAN BE ORDINARY OCCURRENCES IN SPORTS. I HEREBY AGREE TO ACCEPT ANY AND ALL RISKS OF INJURY OR DEATH, AND VERIFY THIS STATEMENT BY MY ELECTRONIC SIGNATURE. FOR MYSELF, SPOUSE, AND CHILD, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, whether known or unknown, and assume full responsibility for my child’s participation in the Activities.

As consideration for being permitted to register for and to participate in the Activities, I hereby release and hold harmless Ignite FC, the organization, volunteers, designated coaches, officers, directors, and other agents, and from all actions or claims that I or my child now or hereafter have WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property incident to my child’s involvement or participation in the Activities, whether arising from the negligence of the releasees or otherwise, to the fullest extent permitted by law. I further agree that this waiver, release and assumption of risks are binding on the personal representative, heirs and assigns of the undersigned.

I further agree to indemnify and to hold Ignite FC, and its volunteers, coaches, officers, directors, and other agents, and advertisers free and harmless from any loss, liability, damage, cost or expense which they may incur as a result of any injury and/or property damage that I or my child may cause or sustain while participating in the Activities.

In case of a medical emergency, I hereby give permission to Ignite FC, its agents, volunteers, coaches, or other agents to order treatment for my child, including any necessary medical treatment or xrays.

I HAVE CAREFULLY READ THIS RELEASE AND FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT BETWEEN ME AND IGNITE FC, AND MY ELECTRONIC SIGNATURE WILL BIND ME THERETO.

I, the registering parent or legal guardian, and on behalf of my minor child identified in this registration, hereby give consent, authorize and release Ignite FC, and its agents, employees, coaches, administrators, officers, or directors to photograph, film, or record our participation, presence or activity at or with Ignite FC, or its Activities. I hereby grant permission to any person associated with Ignite FC to produce, copy, distribute, use, modify and publicly perform and recordings or photography of the minor child contained within the events or Activities, and to use your name, or that of your minor child, along with his/her image and likeness in advertising, promotional materials, and fundraising activities. I agree that no such recordings of photography needs to be submitted to me, or my minor child, for approval. I am willing to release the minor’s image and likeness contained in the recordings of photography into the public domain. I further understand that no monetary compensation will be given to me, or my minor child, for the use of recordings or photography, or publicity rights associated therewith. Finally, I hereby warrant that I am a legally competent adult, who is a parent or legal guardian of the minor, and that I have every right to contract for the minor stated in this release. Upon making my electronic signature, this release shall be binding upon me, and my minor child, and our respective heirs, legal representatives, and assigns.

Fees to Ignite FC (“The Club”) for the player listed above. Anyone who accepts an invitation to join the Club as a player is responsible for, and agrees to pay, the full amount of seasonal fees to the Club. Fees must be paid in full regardless of whether the player listed above plays the entire seasonal year with the club. If the Club has agreed to accept the fees in installments, failure to make a timely payment will be considered a default of player’s terms of registration. Unless satisfactory arrangements have been previously made with the Club to pay in installments, then payment is due upon completion of this registration. If any player remains in active default of payment of any fees after 30 days from notification of the same, the Club may report the player listed above to the Utah Youth Soccer Association and may remove the player in default from the team roster and/or make the player ineligible to play for all or any portion of that seasonal year.

The Club may award scholarships if a player can demonstrate a need for financial assistance per the standards adopted by the Club, and subject to the Club having sufficient scholarship resources available to assist the player. As a scholarship player we expect that you will work hard to be an example and uphold the Club’s core values and principles.

NO REFUNDS WILL BE ISSUED ON THE CLUB FEES ONCE A PLAYER HAS COMMITTED TO AND REGISTERED FOR A TEAM. This NO REFUND policy allows the Club to honor the financial commitments made to the player and team, as well as guarantee the viability of each team. UYSA has a very strict transfer policy, which states players may only transfer into and out of a club between November 15th and Jan 15th. Based on this policy, the Club will only transfer players into and out of the Club during this time. If a player decides to transfer from the club, NO REFUNDS will be issued. Full year club fees (including repayment of scholarship/discount/award monies) must be paid before a player will be released from the Club.

As a condition of the minor child’s participation in Ignite FC, I agree to strictly comply with the Ignite FC Parent Code of Conduct. That Code of Conduct is as follows:

I will refrain from coaching my child from the sideline of any match, training, academy activity, or other official Ignite FC event. Instead, I will only provide positive encouragement to my child’s participation, and I will emphasize that the game is played for fun and to learn for the benefit of the youth, not adults. In furtherance of that requirement, I agree to act as a positive role model by demonstrating positive support for all players, coaches, and game officials regardless of facts or circumstances, including those related to race, creed, color, gender, religion, or ability.

I will refrain from criticizing the game officials, and will respect their authority and decisions during games. I will not engage in any kind of unsportsmanlike conduct with any official, coach, player, parent, or spectator such as booing and taunting, refusing to shake hands, or using profane language or gestures.

I will teach my child to play within the spirit and intentions of the Rules and Laws of the Game. I will never place the value of winning over the safety and welfare of the players. I will demand a sports environment free from drugs, tobacco, and alcohol, and will refrain from their use at all sporting events.

I will NOT encourage, invite, nor recruit any player from an opposing team or club to join my child’s team, outside of the UYSA transfer window.

I will NOT solicit, invite, nor recruit any Ignite FC player or players family, or otherwise seek to promote the interest of another team or club with which my child or family may be associated, outside of the UYSA transfer window.

I understand that if I fail to abide by the aforementioned Code of Conduct, I will be subject to discipline from Ignite FC, or Utah Youth Soccer Association, that could include, but is not limited to the following sanctions: (a) verbal warning by an official, team coach, and/or head of the league organization; (b) forfeiture of a match through an official or team coach; or (c) parental suspension with written documentation to be kept on file with Ignite FC and Utah Youth Soccer Association.

By placing my electronic signature on this document through the registration process, I agree that I have read the Code of Conduct, that I understand the requirements, and agree to abide by the same.
